Arizona Family Health offers cash-pay options for patients who are uninsured or prefer to pay out of pocket. Our current cash-pay pricing is listed below for select office visits, procedures, injections, IV therapy, and add-on services.

Deposit Policy: Certain services may require a non-refundable deposit at the time of scheduling. Any deposit collected will be applied toward the total cost of the service.
Office visit pricing does not include additional procedures, injections, testing, or other services. These are billed separately when performed.
New Patient Establishing Care Visit, All Ages - $200
Established Office Visit, All Ages - $150
Hydration, 500 mL - $150
Each Additional Hydration Bag - $50
IV Add-On
Must be added to a hydration IV.
B12 - $35
Must be added with an office visit.
Hand, Foot, Finger and Toe Injections – $60 per injection
Knee, Shoulder, Hip, Elbow, Wrist and Back – $100 per injection Includes Lidocaine and Kenalog
Must be added with an office visit.
EKG - $50
Breathing Treatment - $50
Rocephin Injection - $75
Kenalog Injection (seasonal allergies/inflammation) - $50
B12 Injection - $50
Ear Lavage - $50 one ear / $75 both ears
ABI - $100
TB Skin Test - $25
Must be added with an office visit.
Osteopathic Manipulation (OMM) 30 minutes - $100
Cryotherapy - $10 per lesion
Suture Placement - $150
Lesion Excision - $200 (does not include cost of pathology report)
Toenail Removal - $125 for first toenail + $50 for each additional toenail
Plantar Wart Removal - $50 per wart
Pricing is subject to change. Some services may require a non-refundable deposit, which will be applied toward the total cost of the service. Additional charges may apply depending on patient needs and services provided.
Good Faith Estimates: Patients who are uninsured or choosing not to use their insurance have the right to receive a Good Faith Estimate of expected charges for scheduled services, as required by federal law. Please contact our office for additional information.
