Arizona Family Health accepts many insurance plans and continues to work on expanding our insurance networks. Because insurance coverage can vary by plan, we recommend contacting our office before your appointment so our team can help review your insurance information and verify your specific benefits.

When searching your insurance provider directory, try looking under both Arizona Family Health and Dr. Christopher Morris / Dr. Chris Morris. Insurance directories may list providers differently depending on the insurance company.

Arizona Family Health currently accepts many plans from the following insurance networks:

Aetna: Commercial, PPO, HMO, EPO, Open Access, Managed Choice, First Health, and many Medicare Advantage plans.

Blue Cross Blue Shield: Many PPO, EPO, HMO, indemnity, statewide/national, and select supplemental plans.

Cigna: PPO, HMO, Open Access, and Local Plus plans.

UnitedHealthcare: Many commercial and Medicare plans, including select EPO, HMO, POS, PPO, Medicare Advantage, AARP Medicare Advantage, and UHC Complete Care Support plans.

Medicare: Please contact our office so we can help verify your specific Medicare coverage.

Medicaid: Please contact our office for insurance coverage confirmation regarding your Medicaid plan.

TRICARE: Please contact our office to verify your specific TRICARE coverage before scheduling.

MultiPlan: Because MultiPlan includes many plan and network variations, please contact our office for coverage confirmation before scheduling.

ASPA Network: Certain ASPA Network plans may be accepted. Please contact our office for coverage confirmation.

Yes. Arizona Family Health is actively pursuing insurance contracting with additional plans. Because insurance credentialing timelines can be unpredictable, we do not provide estimated approval dates.
